Please use the following information sheets shown below to prepare in the event of an outbreak and a mass vaccination clinic is set up in your community. It may be necessary to provide detailed information about your medical history when visiting the vaccination clinic, especially if you have a serious health condition or allergy.
Please fill in the blank forms with as much information as possible for each family member and place it safely in your emergency preparedness kit.
