COVID Vaccination Survey
In an effort to better determine the needs of our community, the Health Department is asking for your assistance in providing valuable information with regards to your Covid-19 vaccination. The information you provide will be kept confidential. It will assist the Health Department in planning how best to provide vaccinations for those still waiting for appointments and those that are unable to visit vaccine sites.
